| What documents should I bring for an appointment?
Bring a list of your children or beneficiaries, their date of birth and address. If you own a home, bring the grant deed (not the mortgage deeds, i.e. deed of reconveyance). Bring a list of your assets. If you are coming for a Medi-Cal appointment, bring in the latest statements, as Medi-Cal requires the actual statements.
